    This course explores practical strategies for translating evidence-based research into meaningful clinical improvements that enhance patient outcomes, support care teams, and create sustainable change within healthcare organizations.

    Introduction: Cardiovascular disease remains a leading cause of morbidity and mortality, yet many modifiable risk factors can be identified before symptoms develop. Routine assessment of blood pressure, lipid levels, glycemic status, lifestyle factors, and family history provides an opportunity for clinicians to recognize elevated cardiovascular risk and initiate preventive interventions earlier.

    Purpose: This quality-improvement initiative evaluated whether implementation of a standardized cardiovascular risk-screening protocol in an outpatient primary care setting could increase identification of patients at elevated risk and improve initiation of appropriate preventive care.

    Results: A total of 1,240 adult patients were evaluated using the standardized screening protocol over a six-month period. Twenty-eight percent were identified as having previously unrecognized moderate-to-high cardiovascular risk. Among these patients, 72% received a documented lifestyle intervention, medication adjustment, additional diagnostic testing, or referral for follow-up care. Documentation of cardiovascular risk assessment increased from 46% before implementation to 87% following implementation of the protocol.

    Conclusion: Standardized cardiovascular risk screening can help clinicians identify patients who may benefit from earlier preventive intervention. Incorporating structured risk assessment into routine care improved documentation and increased initiation of evidence-based preventive strategies. Early identification and management of cardiovascular risk factors may ultimately reduce preventable cardiovascular events and improve long-term patient outcomes.
